3. No Guarantee of Coverage

Villa Health Group does not guarantee:

• Approval for coverage

• Premium pricing

• Eligibility for specific plans

All final determinations are made by the insurance carrier.

4. Client Responsibility

Clients agree to:

• Provide accurate, complete, and truthful information

• Review all plan documents prior to enrollment

• Understand coverage terms, including exclusions and limitations

Villa Health Group is not responsible for outcomes resulting from inaccurate or incomplete information.

5. Ongoing Advisory Support

We provide year-round advisory support; however, clients remain responsible for:

• Paying premiums

• Maintaining active coverage

• Communicating with carriers when necessary

6. Third-Party Carriers

All insurance products are offered through third-party carriers. Villa Health Group is not responsible for:

• Claims decisions or denials

• Policy changes

• Network availability

• Carrier performance or financial stability

7. HIPAA Notice & Use of Information

Villa Health Group may collect personal and health-related information necessary to provide quotes, plan recommendations, and enrollment assistance.

Important HIPAA Disclosure

• Villa Health Group operates as a business associate and/or insurance advisor, not as a healthcare provider.

• Any health-related information you provide is used solely for insurance-related purposes, including underwriting, plan matching, and enrollment.

Authorization to Share Information

By using our services, you authorize Villa Health Group to:

• Collect relevant personal and health information

• Share this information with licensed insurance carriers, underwriting teams, and authorized partners strictly for coverage evaluation and enrollment

Data Handling

We implement reasonable administrative, technical, and physical safeguards to protect your information. However:

• We do not control third-party carrier systems

• We cannot guarantee absolute security of transmitted data

8. No Medical Advice

Villa Health Group does not provide medical advice, diagnoses, or treatment recommendations. Any health-related decisions should be made in consultation with a licensed healthcare provider.

9. Limitation of Liability

To the fullest extent permitted by law, Villa Health Group is not liable for:

• Coverage denials or underwriting outcomes

• Claims processing issues

• Financial losses related to plan selection

• Errors or delays caused by third-party carriers

• Unauthorized access to data beyond our reasonable control

All decisions regarding coverage are the responsibility of the client and the issuing carrier.

10. Website Use

Users agree not to:

• Attempt unauthorized access

• Copy or distribute proprietary content

• Use the site for unlawful purposes
